Hey, lost pilgrim! Sorry for the late response, we've been digging through the game files to learn how the game handles loot.
Put simply, most of it is randomised; the items sold by traders, the items found inside bags, and even the items laying out in the world can be different based on the difficulty you play on - and general RNG.
